we are given the following function:

[tex]f(x)=\sqrt[]{x}[/tex]

The graph of h(x) is the same graph translated 4 units to the left, therefore it must be:

[tex]h(x)=f(x+4)[/tex]

Replacing x for x + 4 in f(x) we get:
